Some construction workers broke open the escape hatch and put this schedule 40 pipe through the opening. The pipe just nicked the CW and blew the doors off and mangled the opening. They're lucky the pipe didn't get pinned under the CW on the way up.

saw a piece of sprinkler pipe punch a hole in a platform once. it was hanging out of the hatch and clipped a spreader beam. also seen an electric pallet jack that knocked the rear gate out of the sill and the car ran down at 800fpm. gate caught a hw sill tore the whole cab off the platform.

I once saw a hospital open the escape hatch of an elevator to use it to transport a ladder that would not fit in the cab. They DID position it to where it would not contact the CW however it is still a stupid move to open the escape hatch to transport items..
