Strange problem, before I get crazy on this thing just wanted to get another opinion, the flashing d indicates and open safety device such as limit switch, chain tension switch or pit switch. I found 5 switches (2 upper limit, 1 lower limit, chain tension and switch up top for an access panel) and all seem to be testing properly as far as opening and closing during actuation. Small burn spot on the main control board on a 38K resistor and above the 15v dc transformer however the resistor tests ok and its still giving the 15v out. Ive done numerous resets as well. Other thing i noticed is one side of the chain is kind of twisted, much more than what I would consider to be normal. The stopped making these units and parts are pretty much non-existent at this point. Any suggestions before I send this board out for checking/repair?

Double check pit switch and car top switch, I've had faulty switches on these before

Thanks for the reply, forgive me im a guy that comes from doing a lot of wheel chair lift installs to try to help out a friend with this elevator, I didnt see any pit switch. I manually cranked the car up enough to get under it, theres nothing under it that the car would come down on, another thing worth noting maybe is that the car dropped below the lowest level, I dont know for certain if it did that on its on or another tech that was there and never came back lowered it down but she said it was never that low before. Same thing on the car top, I didnt see anything, the switches I saw are:

2 Upper
1 lower
Something tied to the chain with i believe is a chain tension switch
switch at the top access panel
2 switches for the accordian doors on both sides of the car
3 door interlocks, one for each level

I truly appreciate the help

by the way, this is the only documentation available on the LEV that I could find on this trouble

1. Safety Device Open
An open safety device, such as the pit or car-top stop switches, a final limit switch, or the slack chain/rope switch, will display a flashing lower case "d". The pump pressure switch on the hydraulic drive will also in the "safety device" circuit

mdm if you can identify the field wiring for the safety circuit coming into the controller pcb, then to prove if it is a faulty switch, place a short across these terminals. I have also had to reset the lift to see if it clears. At least now you can tell if the lift will run.

I should add when placing a short it is only temporary and is only to see if the fault code changes state.

I doubt you have a hydraulic drive, I believe they only shipped a handful before they bought National-Wheel-o-Vator and made the Destiny there default Residential Hydro offering.

What you're describing makes perfect sense, the chain is twisted and the elevator is in the pit, the lift has gone on slack-chain safety.

There is a bracket on the counterweight rails for this, when the chain goes slack it locks onto the rails to prevent it from free-falling. You will have to run the unit up on a service pendant to get it to release then manually reset the slack rope switch.

I would not do this lightly without experience and the proper tools. In reality you need to find out why it went on slack and access. It could have been anything from over-weighting the car to having a mechanical failure causing binding in the counterweights. Let me know where you are located and I may be able to recommend a company.
